Who wrote and put together the books of the bible?
In one word God; the Holy Spirit.
But in detail, apostles and prophets who were moved by the Holy Ghost:

Colossians 4:16
And when this epistle is read among you, cause that it be read also in the church of the Laodiceans; and that ye likewise read the [epistle] from Laodicea.
Although Paul's epistle to Laodicea is lost it seems a common practice to pass/copy around epistles from one church to another.
Paul's epistles were spread in:
1Peter 1
1 Peter, an apostle of Jesus Christ, to the strangers scattered throughout Pontus, Galatia, Cappadocia, Asia, and Bithynia,
2Peter 3
1 This second epistle, beloved, I now write unto you; in which I stir up your pure minds by way of remembrance:
[B]15 And account [that] the longsuffering of our Lord [is] salvation; even as our beloved brother Paul also according to the wisdom given unto him hath written unto you;
16 As also in all [his] epistles, speaking in them of these things; in which are some things hard to be understood, which they that are unlearned and unstable wrest, as [they do] also the other scriptures, unto their own destruction.
All of the books of the New Testament except for 2, James and Jude were written either by the Apostles or under the supervision of an apostle(s) like Luke as he was a disciple of Paul.
I wonder if James and Jude became apostles as the others were martyred or passed away? When Judas died Scripture said let another take his place.
How was the Old Testament put together?

The Old Testament books show (except perhaps the book of Esther) God's reaction to every deed, whether he approves or not. How would man, who wrote an Old testament book know God's heart/reaction?
